    Medical Assistance Program

    The Office of the Vice President provides medical assistance for poor, marginalized, vulnerable, and disadvantaged individuals.

    Clients/Authorized representatives may apply only once every twelve (12) months and may submit the complete and correct documentary requirements through the nearest OVP Satellite Offices. Only applicants with complete requirements shall be processed.

    I. APPLICATION FORM

    • Mayroong kumpletong detalye na may petsa at pirmado ng pasyente o ng kanyang Authorized Representative

    II. MEDICAL/CLINICAL ABSTRACT / MEDICAL CERTIFICATE

    • Pirmado at mayroong License Number ng Doktor (6 Months Validity)

    III. SOCIAL CASE STUDY REPORT / BARANGAY CERTIFICATE OF INDIGENCY

    • Social Case Study Report
    • Naka-address sa OVP
    • May petsa at pirmado ng Licensed Social Worker na hindi lalagpas sa isang (1) taon bago mag-apply para sa assistance

     

    • Barangay Certificate of Indigency
    • Nakapangalan sa pasyente at Authorized Representative kung saan nabanggit ang kanilang relasyon sa isa’t-isa (6 Months Validity)

    IV. VALID IDENTIFICATION CARD (Patient and Representative)

    • Photocopy ng harap at likod ng napiling ID na makikita sa listahan ng OVP
    • Dalhin ang original na ID sa araw ng interview

    MEDICINES

    • Updated Quotation – price quotation o listahan ng presyo ng gamot na nakalagay sa reseta. Maaari itong makuha sa kahit anong drugstore
    • Prescription – may pirma ng physician at petsa (6 Months Validity)
    • Treatment Protocol – pirmado ng physician at may license number na makikita sa medical record (6 Months Validity)

    HOSPITAL BILL

    • Statement of Account – Latest Statement of Account, pirmado ng billing o accounting officer. Kapag hindi tumatanggap ng Guarantee Letter o hindi nagbibigay ng endorsement letter maaring mag Direct Fund Transfer sa pasyente o representative na nakalagay sa Social Case Study Report.

     

    • Endorsement Letter/ Referral Letter – pirmado at naka-address sa OVP. Mayroong contact details ng ospital at Medical Social Worker. Para sa mga Ospital na hindi partner ng OVP, kailangan mayroong bank details ng ospital (Account Name, Account Number, Bank branch, at type of account.)

     

    • Authorization Letter - Kung hindi sa bank account ng pasyente ang gagamitin

     

    • Promissory Note (kapag discharged o nakalabas na sa ospital ang pasyente) – pirmado ng ospital, pasyente o Authorized Representative kabilang ang pirma ng accounting o credit collection officer

    CHEMOTHERAPY, RADIATION THERAPY, BRACHYTHERAPY

    • Treatment Protocol – pirmado ng physician at may license number na makikita sa medical record (6 Months Validity)

    OPERATION/SURGERY
    /TRANSPLANT

    • Updated Quotation – presyo ng operation/surgery na nakalagay sa medical record. Pirmado ng physician at may license number

    IMPLANT (GENERAL)

    • Three (3) Updated Quotation – price quotation o listahan ng presyo ng gamut na nakalagay sa medical record. Maaari itong makuha sa kahit anong supplier

    LABORATORY / DIAGNOSTIC PROCEDURE

    • Request Letter – pirmado at may license number ng Doktor (6 Months Validity)
    • Updated Quotation – price quotation o listahan ng presyo ng laboratory/diagnostic procedure na nakalagay sa medical record. Maaari itong makuha sa ospital o clinic kung saan gagawin ang procedure

    Clients confined in suite rooms, private rooms, or private wards by choice are ineligible. If the confinement in such rooms was not by choice, the Client must request their respective Service Provider to issue a certificate stating that the confinement was beyond the Client's control due to any of the following reasons: 

    1. Emergency cases  
    2. Non-availability of ward services 
    3. Cases of communicable disease requiring isolation, including COVID-19 cases 
    4. Cases requiring intensive care 
    5. Chronic and catastrophic cases requiring prolonged admission
